August 20, 2021 – The Macon County Health Department (MCHD) is offering the following COVID-19 vaccine clinics the week of August 23rd:

Monday, August 23 rd, 2021

• Location: Macon County Health Department

• Time: 1:00 p.m. – 3:30 p.m.

• Vaccine Type: Pfizer, Moderna, and Johnson & Johnson

• Clinic Type: Appointment Only

• Ages: 12 and older

• Please bring photo ID or insurance card if possible.

• Please call 217-423-6988 ext. 1333 or ext. 1324 to make an appointment.

Wednesday, August 25th, 2021 • Location: Richland Community College

• Time: 9:00 a.m. – 12:00 p.m.

• Vaccine Type: Pfizer

• Clinic Type: Walk-in

• Ages: 12 and older

• Please bring photo ID or insurance card if possible.

• Must return to Richland Community College on Wednesday, September 15th at 9:00 a.m. – 12:00 p.m.

Wednesday, August 25th, 2021

• Location: Macon County Health Department Macon County Health Department 1221 E. Condit St

• Time: 1:00 p.m. – 3:30 p.m.

• Vaccine Type: Pfizer

• Clinic Type: Walk-in

• Ages: 12 and older

• Please bring photo ID or insurance card if possible.

• Must return to MCHD Wednesday, September 15th for second dose at 1:00 p.m. – 3:30 p.m.

Thursday, August 26th, 2021

• Location: Millikin University Commons

• Time: 10:00 a.m. – 1:00 p.m.

• Vaccine Type: Johnson & Johnson

• Clinic Type: Walk-in

• Ages: 18 and older

• Please bring photo ID or insurance card if possible.

• Location: Richland Community College • Time: 10:00 a.m. – 1:00 p.m.

• Vaccine Type: Johnson & Johnson

• Clinic Type: Walk-in

• Ages: 18 and older

• Please bring photo ID or insurance card if possible.

Friday, August 27th, 2021

• Location: Macon County Health Department

• Time: 8:30 a.m. – 11:30 a.m.

• Vaccine Type: Pfizer, Moderna, and Johnson & Johnson

• Clinic Type: Appointment Only

• Ages: 12 and older

• Please bring photo ID or insurance card if possible.

• Please call 217-423-6988 ext. 1333 or ext. 1324 to make an appointment. A parent or legal guardian must be present with a minor. If someone other than a parent or legal guardian is planning to bring a child, the Permission/Proxy Form will need to be filled out and signed by the parent or legal guardian. The form must be brought to the clinic.
