• Please bring photo ID or insurance card if possible.
Monday, August 30th, 2021
• Location: Sue Scherer’s Office, 1301 E. Mound Road (Decatur)
• Time: 9:00 a.m. – 12:00 p.m.

• Clinic Type: Walk-In
• Ages: 12 and older
