• Vaccine Type: Pfizer, Moderna and Johnson & Johnson – 1 st, 2nd and booster doses
• Clinic Type: Appointment preferred; walk-ins accepted. • Ages: This clinic is for those 5 years and older.
• Please bring insurance card if possible.
• To schedule an appointment during these hours, persons should email acyrulik@maconchd.org or leave a voicemail at (217) 318-5135 and include their name, date of birth, phone number, and parent name if person is under 18.
• Second dose appointments will be scheduled during your first dose appointment, if needed.
