Tuesday, April 19, 2022
• Location: Macon County Health Department, 1221 E Condit Street, Decatur, IL 62521
• Time: 8:30-11:30 a.m.
• Vaccine Type: Pfizer, Moderna and Johnson & Johnson – 1 st, 2nd and booster doses
• Clinic Type: Appointment preferred; walk-ins accepted.
• Ages: This clinic is for those 5 years and older.
• Please bring insurance card if possible.
• To schedule an appointment, call (217) 423-6988 ext. 1100.
• Second dose appointments will be scheduled during your first dose appointment, if needed.

• 5 months prior for Moderna (18+) and Pfizer (12+)
• 2 months prior for Johnson and Johnson (18+) Persons might qualify for a second booster dose of either Pfizer or Moderna if they are 50 years of age or older and it has been at least four months since their last booster dose.
