Tuesday, January 11, 2022
• Location: Richland Community College, 1 College Park, Decatur
• Time: 9:00 a.m. – 1:00 p.m.
• Vaccine Type: Pfizer, Moderna 1st, 2nd and booster doses • Clinic Type: Appointment only.
• Ages: This clinic is for those 12 years and older.
• Please bring insurance card if possible.
• To schedule an appointment, please call (217) 718-6205
• MUST be able to return to the Macon County Health Department on February 2, 2022 for Pfizer second dose or February 9, 2022 for Moderna second dose.
Wednesday, January 12, 2022
• Location: Macon County Health Department, 1221 E Condit Street, Decatur, IL 62521
• Time: 8:30-11:30 a.m. and 1:00-3:30 p.m.
• Vaccine Type: Pfizer, Moderna and Johnson & Johnson – 1 st, 2nd and booster doses
• Clinic Type: Appointment only. • Ages: This clinic is for those 5 years and older.
• Please bring insurance card if possible.
• To schedule an appointment, please call (217) 718-6205 \
• MUST be able to return on February 2, 2022 for Pfizer second dose or February 9, 2022 for Moderna second dose.
