Upcoming Macon County Health Department vaccination clinics

May 28, 2021 – The Macon County Health Department (MCHD) is offering the following vaccination clinics.

Wednesday, June 2, 2021

• Location: Richland Community College. Please enter the main doors near the flag poles and proceed to the Cafe area.

• Time: 9:00 am to 12:15 pm

• Clinic Type: Walk-in or schedule an appointment

• Vaccination Type: Pfizer

• Ages: 12 years and older

• Must be able to return June 23rd for a second dose.

• If 18 years of age or older, please bring photo ID or insurance card if possible.

Wednesday, June 2, 2021

• Location: MCHD (1221 E. Condit St. Decatur, IL 62521)

• Time: 1:00 pm to 3:15 pm

• Clinic Type: Walk-in

• Vaccination Type: Pfizer

• Ages: 12 years and older

• Must be able to return June 23rd for a second dose.

• If 18 years of age or older, please bring photo ID or insurance card if possible.

Thursday, June 3, 2021

• Location: Richland Community College. Please enter the main doors near the flag poles and proceed to the Cafe area.

• Time: 1:00 pm to 3:15 pm

• Clinic Type: Walk-in or schedule an appointment

• Decatur, IL 62521 (217) 423-6988

• Vaccination Type: Johnson and Johnson (one dose)

• Ages: 18 years and older

• Please bring photo ID or insurance card if possible. For Pfizer clinics:

• PARENT OR LEGAL GUARDIAN MUST BE PRESENT WITH MINOR.

If someone other than a parent or legal guardian is planning to bring the child, the Permission/Proxy Form will need to be filled out and signed by the parent or legal guardian. The form must be brought to the clinic. A person should NOT attend the clinic if they have previously received a COVID-19 vaccination dose of any kind. If a person is pregnant or lactating, a doctor’s note must be provided.
