Decatur Hospitals Seek Community Input Through Health Survey

October 4, 2026 – Decatur Memorial Hospital and HSHS St. Mary’s Hospital are asking residents of Macon and Moultrie counties to participate in a Community Health Survey designed to identify local health needs and priorities.

The survey is part of the hospitals’ community health assessment process and will help guide the development of future Community Health Improvement Plans. Hospital officials say the results help determine where resources, programs and services are most needed.

Previous survey results have contributed to local initiatives, including a Community Health Collaborative focused on prostate cancer screening and awareness, particularly among Black men. Survey findings also helped lead to a social care referral platform connecting patients with resources for food, housing, transportation and other needs.

Memorial Health Director of Community Health Angela Stoltzenburg says community input helps hospitals make informed decisions and develop programs that improve access to care and health outcomes.

HSHS Executive Director of Community Health Kimberly Luz-Mobley says the feedback helps hospitals and community partners focus their efforts on priorities identified by residents.

Residents of Macon and Moultrie counties are encouraged to participate.
