Tuesday, December 7th
• Location: Macon County Health Department, 1221 E Condit Street, Decatur, IL 62521
• Time: 1:00-3:30 p.m.
• Vaccine Type: Pfizer
• Clinic Type: Appointment only.
• Ages: This clinic is for 5-11 year old children only.
• Please bring insurance card if possible.
• MUST be able to return on December 29, 2021, for second dose.
Wednesday, December 8th
• Location: Macon County Health Department, 1221 E Condit Street, Decatur, IL 62521
• Time: 8:30-11:30 a.m. and 1:00-3:30 p.m.
• Vaccine Type: Pfizer, Moderna and Johnson & Johnson – 1 st, 2nd and booster doses
• Clinic Type: Appointment only.
• Ages: This clinic is for those 5 years and older.
• Please bring insurance card if possible.
• MUST be able to return on December 29, 2021 for Pfizer second dose or January 5, 2022 for Moderna second dose.
Thursday, December 9th

• Time: 8:30-11:30 a.m. and 1:00-3:30 p.m.
• Vaccine Type: Pfizer, Moderna and Johnson & Johnson – 1 st, 2nd and booster doses
