• Vaccine Type: Pfizer, Moderna and Johnson & Johnson – 1 st, 2nd and booster doses
• Clinic Type: Appointment only.
• Ages: This clinic is for those 5 years and older.
• Please bring insurance card if possible.

• MUST be able to return on February 9, 2022 for Pfizer second dose or February 16, 2022 for Moderna second dose.
• Persons are eligible for booster doses if they received their primary dose/doses on or before:
➢ 7/19/2021 for Moderna recipients 18 years and older
➢ 8/19/2021 for Pfizer recipients 12 years and older
➢ 11/18/2021 for Johnson & Johnson recipients 18 years and older
